Advanced Deep Plane Facelift Surgery In Washington DC

A deep plane facelift avoids the tight, overfilled look of many past facelifts. The main goal of this procedure is to lift only under the muscle layer, leaving the skin attached to the muscle layers so it will not look too tight. Also, the cheeks are lifted by releasing the tethering points of your face in the deep plane, so adding fat or filler is optional.

Consider a consultation with Maryland’s Expert Facelift Surgeon, Dr. Sheilah Lynch. She uses advanced facelift techniques to provide results that appear completely natural. When people notice your refreshed appearance, it will be without the evidence of surgery. Dr. Lynch is considered one of the top 10 plastic surgeons in Maryland.

A deep plane facelift is facial surgery that lifts and repositions the SMAS layer of connective tissue beneath the facial skin to tighten the cheekbones, jawline, and neck. A deep plane facelift tightens the skin, refines your facial features, and reduces neck laxity. Among today’s facelift options, the deep plane facelift is the most effective, delivering results that can make our patients look years younger.

  • Corrects Deflated Soft Tissues: Significant or rapid weight loss (including from bariatric surgery or GLP-1 medications) causes both deep volume loss and severe skin/tissue laxity.
  • Avoids the “Pulled” Look: Traditional skin-only or limited-SMAS lifts rely heavily on surface tension. When small amounts of subcutaneous fat remain after weight loss, pulling only the skin can create an unnatural, tight appearance. The deep plane technique supports the lift from underneath so the skin redrapes naturally without tension.
  • It Will Restore Midface Volume: It naturally elevates fallen cheek fat pads back to a higher, more youthful position.
  • Long-Lasting Results: Because the structural foundation is anchored deep, the results last significantly longer than mini-lifts.

GLP-1 medications like Ozempic, Wegovy, and Mounjaro have revolutionized weight loss, helping many patients achieve healthier bodies. However, this rapid weight loss has created a new aesthetic challenge: “Ozempic Face.”

Significant weight loss can often leave your face looking hollow, sagging, and prematurely aged. For many patients, their reflection in the mirror no longer matches their new, healthier body. As a board-certified plastic surgeon in Maryland, Dr. Sheilah A. Lynch offers several solutions, including the Deep Plane Facelift.

A deep plane facelift can restore facial fat (with a fat transfer) to restore the volume of your upper cheeks & under eyes, and lips while removing any remaining excess fat from the lower face.

Unlike traditional facelifts that pull your skin tight, a Deep Plane Facelift addresses the root cause. It works primarily beneath the SMAS (the facial muscle layer), releasing the retaining ligaments that sag with weight loss. It vertically repositions the entire mid-face and jawline to their original, youthful position.

Combining Treatments After Weight Loss

Dr. Lynch often pairs a deep plane facelift with selective facial fat grafting (transferring fat to the depleted areas) to restore lost volume while the surgical lift.
  • Facial Fat Grafting: To restore “good” volume in the temples and under-eye area naturally.
  • Neck Lift & Platysmaplasty: To eliminate the “turkey neck” often associated with massive weight loss.

  • Preserving Your Masculine Features: Dr. Lynch will modify the vectors to maintain a strong, angular jawline, sharp cervicomental (neck-to-chin) angles, and avoid an overly tight, rounded, or feminized look.
  • Beard & Hairline Management: Incisions are carefully planned around your sideburns and in front of or behind your ear to prevent shifting beard-bearing skin into non-bearded areas where it is difficult to shave.
  • Thicker Tissue & Bleeding Risk: Men generally have thicker skin, denser fat pads, and a higher tendency to bleed, requiring careful surgical handling.
  • Complementary Procedures: A deep-plane facelift is often paired with a neck lift, platysmaplasty to address jowls and “turkey neck” simultaneously.

An expert deep plane facelift surgeon in Chevy Chase and Easton Maryland, Dr. Lynch can restore your younger appearance, giving you the results and the confidence back for your to enjoy your after weight loss beautiful look.

Frequently Asked Questions

Patients for a deep plane facelift typically include those who wish to refresh or correct the signs of aging in their face and neck, such as sagging skin, deep wrinkles, and loss of youthful contours. Most have:

Visible Signs of Aging

  • Loose or sagging skin in the lower face and neck.
  • Deep folds or creases between the nose and mouth.
  • Loss of jawline definition.
  • Wrinkles or excess fat deposits in the neck area (sometimes referred to as a "turkey neck").

It's important to note: a facelift doesn't treat lip lines, deep wrinkles, creases at the corners of your mouth, or folds at the corners of the nose. A facelift also does not correct the eye area or lift the brow above the eyes.

A deep plane facelift is a more extensive surgery  targeting facial muscles deep into the SMAS layer for a more dramatic result. The entire SMAS layer is lifted by releasing the deep ligaments and then they are repositioned to a higher level without putting too much tension on the skin. The deep plane facelift lifts your lower face, the midface area, including the cheeks, abd neck to create a very youthful and natural appearance.

During the Surgery

Anesthesia

You’ll be given anesthesia to make your comfortable.

Incision Placement

Incisions are typically made in inconspicuous areas:

*Along the hairline at the temples.

*Around the ear, continuing behind the earlobe to the lower scalp.

*When restoring the neck, an additional incision may be made under the chin.

Repositioning and Tightening

* Dr. Lynch will reposition underlying facial tissues and muscles to lift and tighten sagging areas.

*Any excess fat may be sculpted, redistributed, or removed.

*The skin is draped over the newly repositioned contours, and excess skin is trimmed and removed.

Closure

*Incisions are closed with sutures.

*Bandages are applied to protect the treated area and minimize swelling.

Generally, swelling, bruising, and minor discomfort are expected after a facelift, with these symptoms occurring in the first 24 – 48 hours after surgery. You will be given medication to ease any discomfort you experience, along with a specialized compression garment to help minimize swelling. You will have a small drainage tube in place for 24 – 48 hours after surgery, which will then be removed in our office.

Getting Back To Your Routine.

If your recovery goes as expected, you can return to work approximately one week after your surgery. Because swelling and bruising are more noticeable for the first two weeks after surgery, downtime is recommended.

Usually, 6 months after surgery, the swelling will be gone and your results will look astonishing.

A deep plane facelift targets the deeper layers of your facial tissue to include the muscles and ligaments, achieving a more dramatic result. A MACS Lift ( Minimal Access Cranial Suspension) uses a less invasive technique that addresses the superficial skin and in most cases is best for younger patients with minimal sagging, the results are more of a subtle lift with small incisions and a faster recovery time.

Essentially, a deep plane lift is a more comprehensive procedure compared to a MACS lift.

A facelift primarily addresses the lower two-thirds of the face, including the jawline, cheeks, and neck. It does not directly lift or improve under or above your eyelids. If you’re concerned about drooping or aging around the eyes, consider blepharoplasty (eyelid surgery).

Yes, a deep plane facelift typically includes your neck, as this facial surgery will address sagging skin, fat deposits, and muscle laxity in both the lower face and neck.

A facelift does not treat lip lines, deep wrinkles, or creases at the corners of your mouth or at the sides of the nose. Dr. Lynch offers injectables and aesthetic treatments that can help with these concerns.

Dr. Lynch discreetly places incisions within the hairline and inside or behind the ear so they are almost entirely hidden from view. The tissue beneath the skin is lifted into a more youthful position, and then the facial skin is gently re-draped minus the wrinkles. Fat injections are performed afterward if necessary.

As with any surgical procedure, there is always a potential for scarring. However, Dr. Lynch's techniques are designed to minimize the appearance of scars.

Dr. Lynch's facelift techniques focus on subtle, natural enhancement rather than overly tight or the "pulled" look.

Generally, a facelift procedure differs significantly for patients in their 40s versus those in their 60s. The techniques used depend on skin laxity and the condition of the deeper skin layers. Choosing the option of facelift surgery to address many of the concerns related to aging, starts by removing excess skin from your face and neck, restoring skin elasticity, tightening facial tissue, and smoothing wrinkles and folds.

As the world changes and we live in the age of rapid weight loss, we are seeing patients in their late 20s considering a facelift.
